Cannabis can have CNS depressant effects. Combining cannabis with other CNS depressants might result in additive or synergistic effects. A small clinical trial in healthy adults shows that inhaling a high-grade cannabis (Bedrocan International B.V., Veendam, The Netherlands) 100 mg, containing delta-9-tetrahydrocannabinol 21.8% and cannabinol 0.1%, modestly increases subjective feelings of sedation when compared with cannabis alone.

Here's the key thing with this combo: your medication contains phenobarbital, a barbiturate that already calms the nervous system. Cannabis can do the same thing, so using them together may stack those sedating effects. You could feel more drowsy, foggy, dizzy, or slow to react than you'd expect from either one alone. The piece of this combo to watch is the phenobarbital, which is a barbiturate that calms the brain and nervous system. The idea here is that cannabis might slow how fast your body clears phenobarbital, so it could linger longer and build up a bit. In one small study, THC stretched out how long a related barbiturate stayed in the body by about 4 hours.

If that happens, you might feel more of phenobarbital's usual effects: extra drowsiness, grogginess, slowed breathing, dizziness, or trouble concentrating. Cannabis can add its own sedation on top of that. To be fair, this is mostly theoretical and based on pharmacology, not strong human proof. Still, since phenobarbital is potent, please run this by your pharmacist or doctor before mixing the two.

Here's the gist for your Levsin PB type product. The phenobarbital part of this combo is a known liver enzyme booster, meaning it speeds up the CYP3A4 pathway. Since your body uses that same pathway to break down THC, taking cannabis alongside phenobarbital could clear the THC out faster, so you might notice a weaker or shorter-lasting effect from the cannabis than you'd expect.

I want to be straight with you though: this is a theoretical concern based on how these drugs work, not something proven in real people, so it may not be a big deal in practice.
