Fosinopril, Hydrochlorothiazide falls under PHOTOSENSITIZING DRUGS — the group Cilantro interacts with.

Evidence from in vitro research suggests that coriandrin, a constituent of cilantro, has photosensitizing effects. Theoretically, concomitant use with photosensitizing drugs might increase photosensitivity. Some drugs that cause photosensitivity include amitriptyline (Elavil), quinolones (Ciprofloxacin, others), sulfa drugs (Septra, Bactrim, others), and tetracycline.

Here's the short version: the hydrochlorothiazide part of Monopril HCT is already known to make some people's skin more sensitive to sunlight, so you may sunburn faster than usual. Cilantro contains a compound called coriandrin that, in lab dishes, also seems to make skin react more to UV light. Put them together and the worry is a little extra sun sensitivity, meaning easier burns or a rash on exposed skin.

I want to be honest that this is theoretical. It comes from test-tube research, not real patients, and everyday cilantro in your food is very unlikely to cause trouble. Concentrated cilantro supplements or essential oil are a different story. Either way, good sun habits (sunscreen, a hat, shade) cover you. If you have questions, your pharmacist is happy to help.
