Clinafloxacin falls under Quinolone Antibiotics — the group Coconut Water interacts with.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: clinafloxacin belongs to the quinolone family of antibiotics, and quinolones don't mix well with calcium. Coconut water carries a modest amount of calcium (roughly 100 to 300 mg per liter), and that calcium can grab onto the antibiotic in your gut and form a clump your body can't absorb well. The worry isn't a dangerous reaction. It's that less of your antibiotic gets into your bloodstream, which could mean a weaker punch against the infection you're trying to clear.
A few sips probably won't matter much. To be safe, just separate them: take clinafloxacin at least 2 hours before, or 6 hours after, drinking coconut water. A quick check with your pharmacist is always a smart move.
