Diutensen combines two blood pressure lowering ingredients: cryptenamine and methyclothiazide (a thiazide-type water pill). Coconut water has its own mild blood-pressure-lowering effect, so drinking a lot of it alongside this medicine could, in theory, push your pressure down a bit more than expected. That might show up as feeling lightheaded, dizzy on standing, tired, or a little faint. To be clear, this is a theoretical concern based on how things work, not something proven to cause problems for most people.

There's a second wrinkle worth mentioning. Thiazide pills can nudge potassium levels around, and coconut water is naturally rich in potassium. An occasional glass is usually fine, but heavy daily use or concentrated supplements are worth running by your pharmacist or doctor, especially if your kidneys aren't perfect.
