Coconut Water and Gemifloxacin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

Quinolone Antibiotics

Why this applies to Gemifloxacin

Gemifloxacin falls under Quinolone Antibiotics — the group Coconut Water interacts with.

Many quinolone antibiotics, such as ciprofloxacin and levofloxacin, may have reduced absorption when taken with calcium-containing products due to chelation. It is generally recommended to take quinolones at least 2 hours before or 6 hours after consuming oral products that contain calcium. The calcium content of coconut water varies by product but is generally between 100mg-300mg per 1,000mL. Consumption of a small amount of coconut water may be safe but it would be prudent to avoid drinking large amounts at the same time as a quinolone antibiotic. Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about gemifloxacin (Factive): it's a fluoroquinolone, and these antibiotics like to bind to minerals such as calcium. Coconut water carries some calcium (roughly 100 to 300 mg per liter), and when that calcium grabs onto the antibiotic in your gut, less of the drug gets absorbed into your body. That can mean weaker levels fighting your pneumonia, and you don't want a half-strength antibiotic when you're trying to clear an infection.

A few sips probably won't cause trouble, but I wouldn't gulp down a big bottle right alongside your dose. A simple fix is timing: take gemifloxacin at least 2 hours before, or 6 hours after, drinking coconut water. If you're unsure how to space things out, your pharmacist can help you map it out.

About Coconut Water

What is Coconut Water?

Coconut Water
  • Hydration
  • Rehydration after exercise
  • Electrolyte replacement
  • General wellness drink
  • Blood pressure support

Coconut water is a natural drink rich in potassium and other electrolytes that many people use for hydration, especially around exercise. For most healthy people it is a safe, tasty beverage, but it is not a proven medicine and should not replace medical electrolyte solutions for serious dehydration. People with kidney disease or those on potassium-affecting medicines should check with their pharmacist or doctor first.

About Gemifloxacin

What Is Gemifloxacin Used For?

Brand name: Factive

  • Gemifloxacin is used to treat pneumonia. Gemifloxacin may also be used to treat bronchitis but should not be used for this condition if there are other treatment options. Gemifloxacin is in a class of antibiotics called fluoroquinolones. It works by killing bacteria that cause infections.

  • Antibiotics such as gemifloxacin do not work for colds, flu, or other viral infections. Using antibiotics when they are not needed increases your risk of getting an infection later that resists antibiotic treatment.
