Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with coconut water and your human insulin: the worry is that both can push your blood sugar in the same direction, down. Insulin already moves sugar out of your blood, and some early lab and animal studies hint that coconut water might lower glucose a bit too. Stack them and, in theory, you could dip lower than expected, with signs like shakiness, sweating, a racing heart, or feeling lightheaded.

That said, I don't want to overstate this. The evidence is weak (mostly animal research), the interaction is rated minor and unlikely, and a normal glass to rehydrate probably won't cause trouble for most folks. If you drink it regularly, just keep checking your sugars and mention it to your pharmacist or doctor before changing anything.
