Diutensen-R combines two blood pressure lowering ingredients: methyclothiazide (a thiazide water pill) and reserpine. Coconut water, on its own, may nudge blood pressure down a little too. So the idea here is simply additive. Drinking a lot of it alongside your medicine could push your pressure a bit lower than expected. That might show up as lightheadedness, dizziness when you stand up, feeling tired, or even fainting.

To be fair, this is a theoretical concern based mainly on how things work, not strong proof. An occasional glass is unlikely to cause trouble for most people. The bigger thing to know is that this thiazide can lower potassium, and coconut water adds potassium, so the two may roughly balance out. If you drink it often, it's worth a quick chat with your pharmacist or doctor.
