Here's the short version: this one is mostly theoretical. Some animal studies hint that coconut milk might nudge blood sugar a little lower or bump up insulin. If that effect carried over to people on metformin, it could in theory add to metformin's sugar-lowering work and slightly raise the chance of low blood sugar (think shakiness, sweating, dizziness, or feeling foggy).

In real life, eating coconut as food, whether it's the flesh, milk in a curry, or a splash of coconut water, is very unlikely to cause trouble. There's no strong human evidence here. If you're using concentrated coconut products regularly or notice your numbers running low, just keep an eye on your readings and mention it to your pharmacist or doctor before changing anything.

Metformin is used to improve blood sugar (glucose) control in people with type 2 diabetes mellitus. It is always used together with diet and exercise, not instead of them.

Metformin hydrochloride tablets and oral solution are approved for adults and children 10 years and older. Metformin hydrochloride extended-release tablets — including brands such as Fortamet, Glumetza, and Metformin Hydrochloride Extended Release — are approved for adults only.
