Why this applies to Bedaquiline

Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cubebs interacts with.

Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.

Here's the short version for bedaquiline (Sirturo). Your body clears this TB medicine using a liver enzyme called CYP3A4. Some compounds in cubeb berries have slowed that same enzyme down, but only in lab dish studies, not in actual people. So this is a theoretical concern, not a proven one.

If it did happen, the worry would be that bedaquiline clears more slowly and builds up, which could nudge up side effects. The big one to watch with bedaquiline is a heart rhythm change called QT prolongation. Using cubeb as an occasional cooking spice almost certainly won't matter. Concentrated cubeb extracts or oils are the bigger question mark, so check with your pharmacist or TB doctor before adding those.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
