Omeprazole, Sodium Bicarbonate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cubebs interacts with.

Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.

Here's the gist for your Konvomep, which pairs omeprazole with sodium bicarbonate. Lab studies suggest some compounds in cubeb berries can slow down a liver enzyme called CYP3A4. Omeprazole is partly handled by that enzyme (though it leans more on a different one, CYP2C19), so in theory cubebs could let a little more omeprazole linger in your system. That might mean a slightly stronger effect or side effects like headache, nausea, or stomach upset.

That said, this is only test-tube research and hasn't been shown to happen in real people. A pinch of cubeb as a spice is very unlikely to matter. Concentrated extracts or oils are where I'd be more cautious, so check with your pharmacist or doctor before using those regularly.

Theoretically, due to reports that cubebs increases stomach acid, cubebs might decrease the effectiveness of PPIs. PPIs include omeprazole (Prilosec), lansoprazole (Prevacid), rabeprazole (Aciphex), pantoprazole (Protonix), and esomeprazole (Nexium).

Here's the simple version. Your medication, Konvomep, combines omeprazole (a proton pump inhibitor that quiets down stomach acid) with sodium bicarbonate (a buffer that helps the omeprazole survive your stomach). The whole point is to lower acid and ease heartburn, reflux, or ulcer pain. The idea behind this interaction is that cubebs may nudge your stomach to make more acid, which could work against what your medicine is trying to do.

Honestly, this one is pretty theoretical and based on weak, anecdotal reports, so I wouldn't lose sleep over it. A little cubeb as a kitchen spice is unlikely to matter. Concentrated extracts or oils are a different story. If your reflux symptoms creep back, chat with your pharmacist or doctor before changing anything.
