Omeprazole, Sodium Bicarbonate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grapefruit interacts with.

Grapefruit juice can increase levels of drugs metabolized by CYP3A4.
Clinical research shows that grapefruit juice can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. When taken orally, effects of grapefruit juice on CYP3A4 levels appear to last at least 48 hours. Grapefruit's ability to inhibit CYP3A4 has even been harnessed to intentionally increase levels of venetoclax, which is metabolized by CYP3A4, in an elderly patient with acute myeloid leukemia who could not afford full dose venetoclax. The lower dose of venetoclax in combination with grapefruit juice resulted in serum levels of venetoclax in the therapeutic reference range of full dose venetoclax and positive treatment outcomes for the patient.
Professional consensus recommends the consideration of patient age, existing medical conditions, additional medications, and the potential for additive adverse effects when evaluating the risks of concomitant use of grapefruit juice with any medication metabolized by CYP3A4. While all patients are at risk for interactions with grapefruit juice consumption, patients older than 70 years of age and those taking multiple medications are at the greatest risk for a serious or fatal interaction with grapefruit juice.

Here's the good news first: omeprazole isn't really a CYP3A4 drug. Your body clears it mainly through a different liver enzyme called CYP2C19 (with some help from CYP3A4), so grapefruit's main trick of blocking CYP3A4 doesn't have a big effect on it. At most, grapefruit might nudge omeprazole levels up a little, which in practice tends to be minor.

If anything happened, you might notice slightly stronger acid-blocking effects or mild stomach upset, but this is not a dangerous combination for most people. The sodium bicarbonate part isn't affected by grapefruit at all. Still, since grapefruit interacts with so many medicines, it's worth a quick chat with your pharmacist about everything else you take.

Theoretically, grapefruit juice might increase levels of drugs metabolized by CYP2C19.
In vitro research suggests that grapefruit juice might inhibit CYP2C19 enzymes. Also, a small clinical study shows that taking grapefruit juice with clopidogrel, an antiplatelet prodrug that is metabolized primarily by CYP2C19, decreases plasma levels of the active metabolite and impairs the antiplatelet effect of clopidogrel. This effect is likely due to grapefruit-induced inhibition of CYP2C19.

Here's the deal with omeprazole (the active ingredient in Konvomep, paired with sodium bicarbonate). Your body relies partly on an enzyme called CYP2C19 to break omeprazole down. Grapefruit appears to slow that enzyme, mostly based on lab studies and weaker evidence, so in theory more omeprazole could linger in your system. Omeprazole isn't a prodrug, so this would mean a bit more drug effect, not less.

The good news is omeprazole is already a very safe, well-tolerated medicine, and a small bump usually just means slightly more of the usual stuff like headache, nausea, or loose stools. This is theoretical and nothing to panic over, but if you enjoy grapefruit regularly, it's worth a quick chat with your pharmacist.

Here's the short version: this is a theoretical concern, not something doctors have actually seen happen in real patients. The idea is that grapefruit might block a liver enzyme called CYP2C9, and omeprazole is partly handled by that pathway. If grapefruit slowed that enzyme down, a little more omeprazole could hang around in your body. In practice, that might mean slightly stronger or longer-lasting effects, things like headache, mild stomach upset, or gas.

Honestly, this signal comes only from test-tube research, so I wouldn't lose sleep over an occasional grapefruit. Omeprazole is also a pretty forgiving medicine. Still, if you eat grapefruit often or notice new side effects, it's worth a quick chat with your pharmacist or doctor before changing anything.
