Osilodrostat Phosphate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cubebs interacts with.

Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.

Here's the gist for Isturisa (osilodrostat), the medicine you take to lower cortisol in Cushing's disease. Your liver partly clears it using an enzyme called CYP3A4. In test-tube studies, certain compounds in cubeb berries can slow that enzyme down. If that happened in your body, osilodrostat could build up a bit and hit harder than expected.

In practice that might mean more of its usual effects, like nausea, fatigue, headache, dizziness, swelling, or cortisol dropping too low (weakness, low blood pressure, low sodium). Keep in mind this is only laboratory evidence, not something seen in people, and a sprinkle of cubeb as a spice is unlikely to matter. Concentrated extracts or oils are the real question, so check with your pharmacist or doctor before using them.
