Clozapine falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Dendrobium interacts with.

The dendrobium constituent dendrobine is reported to have convulsant effects. Theoretically, combining dendrobium with pro-convulsant drugs might increase the risk of seizure. Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

Here's the thing to know about clozapine: among antipsychotics, it's well known for lowering the seizure threshold on its own, meaning it can make seizures a little more likely, especially at higher doses. Dendrobium contains a compound called dendrobine that, in lab and animal work, has shown convulsant (seizure-promoting) activity. So in theory, pairing the two could nudge that seizure risk a bit higher.

I want to be honest about the evidence here: this is theoretical, based on pharmacology rather than real patient reports, so I don't want to alarm you. Still, because clozapine already carries its own seizure risk, I'd treat this combo cautiously. Please check with your pharmacist or doctor before adding Dendrobium, and never adjust your clozapine on your own.

What Is Clozapine Used For?

Clozapine is used for two specific situations. First, it is prescribed for people with severe schizophrenia who have already tried other antipsychotic medications and have not gotten adequate relief. It is reserved for this group because of its serious risks.

Second, clozapine is used to lower the chances of repeated suicidal behavior in people with schizophrenia or schizoaffective disorder who are considered to be at ongoing, chronic risk. It is not a first-choice medication — it is used when the need is serious and other options have fallen short.
