CLOZAPINE (Clozaril)
Cola nut contains caffeine. Caffeine can increase the effects and toxicity of clozapine. Caffeine doses of 400-1000 mg daily inhibit clozapine metabolism. Clozapine is metabolized by cytochrome P450 1A2 (CYP1A2). Researchers speculate that caffeine might inhibit CYP1A2. However, there is no reliable evidence that caffeine affects CYP1A2. There is also speculation that genetic factors might make some patients more sensitive to the interaction between clozapine and caffeine.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation: cola nut gets its kick from caffeine, and caffeine may slow down the liver enzyme (called CYP1A2) that clears clozapine from your body. When that enzyme is blocked, clozapine can build up to higher levels than expected. That could mean stronger side effects, things like heavy sedation, dizziness, drooling, a faster heartbeat, low blood pressure, or even a worsening of your symptoms.
I want to be honest, though: this is mostly theoretical, and the evidence is on the weaker side. It seems to matter more with concentrated caffeine, roughly 400 mg or more a day, than with a small amount. Since clozapine is a medication that's watched closely anyway, please run any cola nut or caffeine supplements past your pharmacist or doctor first.
