Here's the short version: cola nut's main active ingredient is caffeine, and the worry is that caffeine might nudge your blood sugar a little, either up or down. Since inhaled insulin (Exubera) works to bring your blood sugar down at mealtimes, any extra swing on top of it could make your readings less predictable. In practice this is rated as a minor, unlikely interaction, and the evidence is mixed, so I wouldn't lose sleep over it.

That said, if you use cola nut regularly and notice more lows (shakiness, sweating, dizziness) or unexpected highs, that's worth mentioning. Keep an eye on your glucose checks, and chat with your pharmacist or doctor before changing anything with your insulin or your supplements.
