Theoretically, concomitant use might increase stimulant adverse effects.
Cola nut contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Severity Moderate Likelihood Probable Evidence C

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: cola nut is basically a caffeine source, and methylphenidate (Ritalin, Concerta) is already a stimulant. When you put two stimulants together, their effects can stack up. This isn't about one blocking or speeding up the other in your liver. It's more straightforward than that, both of them rev up your nervous system, so the side effects can add up.

What that can feel like with methylphenidate: a faster or pounding heartbeat, higher blood pressure, jitteriness, anxiety, and trouble sleeping. This is mostly expert caution rather than hard proof, and a little cola in a beverage is unlikely to be a big deal. But concentrated cola nut supplements are a different story. If you notice your heart racing or you feel wired, check in with your pharmacist or doctor before changing anything.

From our pharmacist’s overview on the Methylphenidate page

“Methylphenidate (Ritalin, Concerta, Metadate) is itself a stimulant, and that one fact shapes most of this list. The largest cluster of interacting supplements adds a second stimulant on top of it, from ephedra and its relatives to everyday caffeine sources like coffee and green tea, which can push heart rate and blood pressure higher…”

Cola nut is a caffeine-containing seed from West Africa used mainly as a natural stimulant for energy and alertness. Most of its effects come from caffeine, and strong human evidence for specific health benefits is limited. Because of its caffeine content, people sensitive to stimulants and those who are pregnant or have heart conditions should be cautious.
