Here's the thing to keep in mind with this combination. Cola nut is basically a natural source of caffeine, and your body breaks down caffeine and theophylline using the same liver pathway. When you load up on caffeine from cola nut supplements, it can compete for and slow down that pathway, which may let theophylline build up higher than expected in your blood.

That matters because theophylline has a narrow safety window, meaning small changes can tip you into trouble. Higher levels can cause jitteriness, a racing or pounding heart, nausea, trouble sleeping, or in more serious cases tremors or irregular heartbeats. The evidence here is on the theoretical side and mostly tied to large caffeine doses, not a single soda. Still, with a drug this sensitive, please check with your pharmacist or doctor before adding cola nut.

Theophylline — available as extended-release capsules (Theo-24), extended-release tablets, and oral solution (Elixophyllin) — is used to treat symptoms and reversible airflow obstruction caused by chronic asthma, emphysema, and chronic bronchitis.

All supported formulations share these indications. Theophylline is intended for patients with ongoing or recurring symptoms, not for sudden (acute) breathing attacks, which need faster-acting treatments.
