Here's the situation with cola nut and your dipyridamole. Dipyridamole already works to keep blood from clotting too easily, which is exactly why you're taking it after your valve replacement. Cola nut is mostly caffeine, and in lab dishes caffeine has shown a mild ability to make platelets a little less sticky too. Stacked together, the theory is they could nudge your bleeding risk up slightly.

I want to be honest with you though: this is purely theoretical, based on test tube work, and it has never actually been reported in real people. A cup of cola-nut tea or a soda is very unlikely to cause trouble. If you're taking concentrated cola nut supplements for energy, just mention it to your pharmacist or doctor, and watch for things like easy bruising or bleeding gums. Don't change your dipyridamole on your own.

Theoretically, cola nut might decrease the vasodilatory effects of dipyridamole and interfere with its use prior to stress testing.
Cola nut contains caffeine. Caffeine may inhibit dipyridamole-induced vasodilation. It is recommended that methylxanthines and methylxanthine-containing products, such as cola nut, be stopped 24 hours prior to pharmacological stress tests. Methylxanthines appear more likely to interfere with dipyridamole than with adenosine-induced stress testing.

Here's the key thing with cola nut and dipyridamole: cola nut is loaded with caffeine, and caffeine basically works against one of dipyridamole's main jobs. Dipyridamole widens your blood vessels (that's the effect doctors lean on during a pharmacological stress test of your heart). Caffeine blocks that vessel-widening action, so the two can cancel each other out.

In everyday use for clot prevention this isn't usually a big deal, but it matters a lot around a stress test. If caffeine from cola nut is in your system, the test can give misleading results. That's why most clinics ask you to stop caffeine and caffeine-containing products like cola nut for about 24 hours beforehand. Please check with your doctor or pharmacist about timing before any scheduled testing.

What Is Dipyridamole Used For?

Brand names: Persantine, Dipyridamole, Persantin, Persantin Injection, Persantin Retard

Dipyridamole tablets are used alongside warfarin (a blood thinner) to help prevent dangerous blood clots from forming after cardiac valve replacement surgery. It is not used alone for this purpose — it works as an add-on to anticoagulant therapy.

Dipyridamole Injection is used as an alternative to exercise during thallium myocardial perfusion imaging — a type of heart scan that checks blood flow through the coronary arteries. It is given to patients who are unable to exercise adequately for the test.
