Here's the gist with nateglinide (Starlix) and cola nut. Nateglinide works fast to nudge your pancreas to release insulin around mealtimes, which lowers your blood sugar. Cola nut is basically a caffeine source, and caffeine's effect on blood sugar is genuinely mixed in the research. Some reports suggest it can bump sugar up a bit, others suggest the opposite, so the direction isn't clear.

The practical takeaway: this is rated minor and unlikely, and the evidence is on the weaker side. A cup of cola-flavored or caffeinated drink probably won't throw your numbers off. With concentrated cola nut supplements, just keep an eye on your home glucose readings and watch for shakiness or feeling off. Nateglinide is not for type 1 diabetes, and it is not used to treat diabetic ketoacidosis (a serious complication where the blood becomes dangerously acidic).
