Here's the gist with cola nut and your alogliptin (Nesina). This isn't a direct chemical clash. Alogliptin lowers blood sugar by helping your body release more of its own insulin, and cola nut doesn't change how your body breaks the medicine down. The concern is simply the caffeine in cola nut. Caffeine can nudge blood sugar in either direction, and the reports on this are honestly mixed and weak.

So the practical worry is that lots of caffeine might make your numbers a little less predictable while you're working to control them. A cup of coffee or a normal soda is unlikely to matter much. Concentrated cola nut supplements taken daily are more of a gray area. If you decide to use them, check your blood sugar a bit more closely and let your pharmacist or doctor know.
