Phenobarbital, Phenytoin falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Eurycoma Longifolia interacts with.

Theoretically, Eurycoma longifolia might increase levels of CYP2C19 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2C19 enzymes. This effect has not been reported in humans.

Here's the gist with tongkat ali and your seizure medicines. Phenytoin and phenobarbital are partly cleared by a liver enzyme called CYP2C19. In a test-tube study, an extract of this herb slightly slowed that enzyme down. The worry, in theory, is that your drug levels could drift a little higher than usual.

I want to keep this in perspective. This is only lab data, and it has never actually been seen in people, so the real-world risk looks low. Still, phenytoin especially has a narrow safety window, so even small shifts can matter. If levels climbed, you might notice things like dizziness, unsteady walking, slurred speech, drowsiness, or double vision. Please loop in your pharmacist or doctor before adding it, and don't change your dose on your own.
