Trandolapril, Verapamil falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Eurycoma Longifolia interacts with.

Theoretically, Eurycoma longifolia might increase levels CYP1A2 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP1A2 enzymes. This effect has not been reported in humans.

If you take Tarka (the combo of trandolapril and verapamil) and you're curious about tongkat ali, here's the honest picture. In a lab dish, eurycoma extract very weakly slowed down an enzyme called CYP1A2. The thinking is that blocking it could let certain drugs build up a little. But this has never actually been seen in people, so it stays in the theoretical column.

On top of that, verapamil and trandolapril aren't mainly handled by CYP1A2 anyway (verapamil leans on CYP3A4), so I wouldn't expect a real-world change in blood pressure, heart rate, or side effects like dizziness or swelling. Still, since blood pressure meds matter, run any new supplement past your pharmacist or doctor first.
