Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Hyzaar combines two blood pressure medicines: losartan (which relaxes blood vessels) and hydrochlorothiazide (a water pill). Goldenseal contains berberine, and on paper berberine may have a mild blood pressure lowering effect of its own. So the concern here is simple addition: pairing goldenseal with a medicine that already lowers your pressure could, in theory, push it down a little too far.

I want to be honest about how soft this evidence is. It comes mostly from animal studies and from berberine paired with a different drug, not goldenseal with Hyzaar itself. If your pressure did dip, you might feel lightheaded, dizzy when standing, or unusually tired. That is rarely dangerous, but please check with your pharmacist or doctor before adding goldenseal, and don't change your Hyzaar on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Hydrochlorothiazide, Losartan Potassium also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the wrinkle with Hyzaar and goldenseal. Hyzaar pairs two medicines: hydrochlorothiazide (a water pill) and losartan (a blood pressure drug). Losartan is actually a prodrug, meaning your liver has to convert it into its active form to do most of its work. Some of that activation runs through CYP3A4, the same enzyme goldenseal tends to block.

So in theory, goldenseal could slow that conversion and leave you with a bit less active losartan, possibly making your blood pressure control weaker. The honest truth is the evidence here is modest, and the effect is more likely with concentrated supplement doses than a casual dose. Before adding goldenseal, check with your pharmacist or doctor and keep an eye on your blood pressure readings.

Theoretically, goldenseal might decrease the conversion of losartan to its active form.
Goldenseal contains berberine. A small clinical study shows that berberine inhibits cytochrome P450 2C9 (CYP2C9) activity and reduces the metabolism of losartan. However, this effect has not been reported with goldenseal.

Your Hyzaar is a combo pill: hydrochlorothiazide (a water pill) plus losartan (a blood pressure medicine). Here's the wrinkle with goldenseal. Losartan is actually a prodrug, meaning your liver has to convert it (using an enzyme called CYP2C9) into the active form that does most of the blood pressure lowering. Goldenseal's berberine can slow that enzyme down, so in theory you might make less of the active drug and get a bit less blood pressure control.

I want to be honest about the evidence, though. This is only theoretical, based on a small study using berberine itself, not goldenseal, and it hasn't actually been seen with the herb. The hydrochlorothiazide part isn't affected. If you take goldenseal supplements regularly, just mention it to your pharmacist or doctor and keep an eye on your blood pressure readings.
