Why this applies to Rabeprazole

Rabeprazole falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Goldenseal interacts with.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the gist: goldenseal contains alkaloids (berberine and hydrastine) that, in test-tube studies, can mildly slow down a liver enzyme called CYP2C9. Rabeprazole is one of the medicines that this enzyme helps break down. In theory, if goldenseal slows that breakdown, a bit more rabeprazole could linger in your system, which might mean slightly stronger effects or a higher chance of minor side effects like headache, nausea, or stomach upset.

The honest part: this concern comes only from lab research, not from real people taking both together. So I wouldn't lose sleep over it. Rabeprazole is generally well tolerated and isn't a high-risk drug. Still, if you're planning to take concentrated goldenseal supplements regularly, it's worth a quick chat with your pharmacist or doctor first.

Rabeprazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist for rabeprazole (Pariet): your liver partly uses an enzyme called CYP3A4 to clear it out. Goldenseal can slow that same enzyme down. In theory, that means a little less rabeprazole gets broken down, so more can hang around in your blood.

The good news is that rabeprazole is generally a forgiving, well-tolerated medicine, so a modest bump usually isn't dramatic. If anything, you might notice mild side effects a bit more, like headache, nausea, or gassiness. This is based on limited research, not strong proof, so I wouldn't panic. Still, because goldenseal supplements are far more concentrated than any food, it's worth a quick chat with your pharmacist or doctor before combining them.

What Is Rabeprazole Used For?

Rabeprazole delayed-release tablets treat a range of acid-related conditions in adults, including healing and maintenance of erosive or ulcerative GERD (gastroesophageal reflux disease), symptomatic GERD, duodenal ulcers, and pathological hypersecretory conditions such as Zollinger-Ellison syndrome. They are also used alongside antibiotics (amoxicillin and clarithromycin) to eradicate H. pylori bacteria and reduce the risk of duodenal ulcer coming back.

In adolescents 12 years and older, rabeprazole delayed-release tablets are approved for short-term treatment of symptomatic GERD. Both AcipHex and Rabeprazole Sodium delayed-release tablets share this indication set for adults and adolescents 12 and older.
