Pemigatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the situation with goldthread and your Pemazyre. Pemigatinib is cleared from your body largely by a liver enzyme called CYP3A4. Goldthread contains berberine, which lab studies suggest might slow that same enzyme down. If that happens, your body could clear pemigatinib more slowly, letting levels creep up. That could mean stronger side effects, things like mouth sores, dry eyes or mouth, diarrhea, joint pain, fatigue, or changes in your blood phosphate levels.

The honest part: this interaction is theoretical right now, based on how berberine behaves in the lab, not actual reports in people. Still, because pemigatinib is a serious cancer drug, I'd play it safe. Please check with your oncologist or pharmacist before adding goldthread.

About Pemigatinib

What Is Pemigatinib Used For?

Brand name: Pemazyre

Pemazyre (pemigatinib) is used to treat adults with a type of bile duct cancer called cholangiocarcinoma that cannot be surgically removed (locally advanced or metastatic) and has already been treated with at least one prior therapy. It is only for tumors that carry a specific gene change — an FGFR2 fusion or rearrangement — confirmed by an FDA-approved test. This approval is based on tumor response rates, and further studies are underway to confirm long-term benefit.

Pemazyre is also used to treat adults with relapsed or refractory myeloid/lymphoid neoplasms (MLNs) — blood or lymph system cancers that have stopped responding to or come back after prior treatment — when those cancers carry an FGFR1 rearrangement gene change.
