CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Pemigatinib
Pemigatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Turmeric interacts with.
In vitro and animal research show that turmeric and its constituents curcumin and curcuminoids inhibit CYP3A4. Also, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking turmeric and cancer medications that are CYP3A4 substrates, including everolimus, ruxolitinib, ibrutinib, and palbociclib, and bortezomib. In another case report, a transplant patient presented with acute nephrotoxicity and elevated tacrolimus levels after consuming turmeric powder at a dose of 15 or more spoonfuls daily for ten days prior. It was thought that turmeric increased levels of tacrolimus due to CYP3A4 inhibition.
Conversely, other in vitro research suggests that turmeric induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. An animal model suggests that induction of CYP3A4 occurs after daily curcumin use for 1 week. However, the induction of CYP3A4 by turmeric has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body uses a liver enzyme called CYP3A4 to help clear pemigatinib (Pemazyre). Turmeric, especially in concentrated supplement doses, may block that enzyme. If that happens, pemigatinib could build up higher than intended, which may mean stronger side effects like mouth sores, dry eyes or vision changes, diarrhea, fatigue, low phosphate problems, or changes in your nails and skin.
I'll be honest with you, this concern comes mostly from lab and animal studies, plus a few reports with other cancer drugs, so it isn't proven in people taking pemigatinib. Sprinkling turmeric on food is unlikely to matter much. Still, since this is a serious cancer medicine, please run any turmeric supplement past your oncologist or pharmacist first.

About Pemigatinib
What Is Pemigatinib Used For?
Brand name: Pemazyre
Pemazyre (pemigatinib) is used to treat adults with a type of bile duct cancer called cholangiocarcinoma that cannot be surgically removed (locally advanced or metastatic) and has already been treated with at least one prior therapy. It is only for tumors that carry a specific gene change — an FGFR2 fusion or rearrangement — confirmed by an FDA-approved test. This approval is based on tumor response rates, and further studies are underway to confirm long-term benefit.
Pemazyre is also used to treat adults with relapsed or refractory myeloid/lymphoid neoplasms (MLNs) — blood or lymph system cancers that have stopped responding to or come back after prior treatment — when those cancers carry an FGFR1 rearrangement gene change.
