Dextromethorphan, Methscopolamine, Phenylephrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Grape interacts with.
In vitro evidence suggests that grape seed extract might inhibit CYP2D6 enzymes. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This cold and cough combo contains dextromethorphan, which your body breaks down using a liver enzyme called CYP2D6. In lab dishes, grape seed extract has been shown to slow that same enzyme down. If that held true in people, dextromethorphan could clear more slowly and build up a bit, making side effects like drowsiness, dizziness, or feeling foggy more likely. The other two ingredients aren't strongly tied to this pathway.
Here's the reassuring part: this is only theoretical, based on test-tube studies, and it hasn't actually been seen in real patients. Eating grapes or drinking grape juice almost certainly won't matter. If you're taking a concentrated grape seed supplement, just mention it to your pharmacist so they can keep an eye on things.

In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

This is one of those interactions that looks scarier on paper than it is in real life. One ingredient in your cold combo, dextromethorphan (the cough suppressant), is partly broken down by a liver enzyme called CYP3A4. Grape seed extract may touch that same enzyme, but here's the catch: lab studies say it might block it, while animal studies say it might speed it up. The findings flat-out disagree, and nothing has actually been seen in people.
If anything happened, blocking that enzyme could let a little more dextromethorphan build up, which might mean more drowsiness or dizziness. But this is theoretical only. Eating grapes won't be an issue, and even concentrated supplements are unlikely to cause real trouble. Still, it's worth a quick chat with your pharmacist before pairing them.
