Selpercatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grape interacts with.

It is unclear if grape seed extract inhibits or induces CYP3A4; research is conflicting.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Here's the short version: your cancer medicine selpercatinib (Retevmo) is cleared by a liver enzyme called CYP3A4, and concentrated grape seed extract may tug on that same enzyme. The catch is that the lab and animal studies disagree. Some test-tube work hints it could block the enzyme (which would let selpercatinib build up, raising the chance of side effects like high blood pressure, liver enzyme changes, diarrhea, or tiredness), while animal data hint it could speed up the enzyme (which could make the drug less effective).

Because the direction is genuinely unclear and this has not been seen in real people, I wouldn't panic. Eating grapes is fine. But before starting a grape seed supplement alongside a cancer drug like this, please check with your oncology pharmacist or doctor first.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
