Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP2D6 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP2D6. This interaction has not been reported in humans.

Here's the gist for donepezil (Aricept): it's partly cleared by a liver enzyme called CYP2D6. In lab dish studies, concentrated hibiscus extract slowed that enzyme down a bit. If that happened in your body, donepezil could clear a little slower and build up slightly, which in theory might bump up its usual side effects like nausea, diarrhea, dizziness, a slower heart rate, or trouble sleeping.

I want to be honest, though, this is purely theoretical. It has never actually been reported in people, and a cup of tart hibiscus tea is a very different thing from a strong supplement capsule. I wouldn't lose sleep over it, but if you plan to take concentrated hibiscus regularly, just give your pharmacist or doctor a quick heads-up first.

I want to keep this in perspective. This is a theoretical concern based only on test-tube work, never actually seen in humans, and it's rated minor.
