Most clinical evidence suggests that taking Hibiscus sabdariffa reduces systolic and diastolic blood pressure. In animal research, Hibiscus sabdariffa increased the hypotensive effects of single doses of losartan and amlodipine.

Your combo pill pairs hydrochlorothiazide (a water pill) with metoprolol (a beta blocker), and both are already working to bring your blood pressure down. Hibiscus, especially as a daily tea or concentrated supplement, has its own mild blood-pressure-lowering effect. Stack them together and the numbers could dip a little lower than expected.

This concern is mostly theoretical, based on how these things work and a few animal studies, not strong human proof. Still, it's worth knowing the signs of pressure dropping too far: lightheadedness, dizziness when you stand, tiredness, or feeling faint. An occasional cup of hibiscus tea is unlikely to cause trouble, but regular concentrated doses deserve a quick chat with your pharmacist or doctor first.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Hydrochlorothiazide, Metoprolol also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP2D6 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP2D6. This interaction has not been reported in humans.

Your combo pill pairs a water pill (hydrochlorothiazide) with metoprolol, a beta-blocker. The part that matters here is the metoprolol, which your body clears using a liver enzyme called CYP2D6. In lab dish studies, hibiscus extract slowed that enzyme down. If that happened in real life, metoprolol could build up a bit, possibly nudging your heart rate or blood pressure lower than expected and bringing on tiredness, lightheadedness, or cold hands.

The reassuring news is that this is only a theoretical concern from test-tube work, never actually reported in people, and it's rated minor. An occasional cup of tart hibiscus tea is very unlikely to cause trouble. Concentrated hibiscus supplements are where I'd be more cautious, so check with your pharmacist or doctor before starting one.
