Ibuprofen, Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Hibiscus Sabdariffa interacts with.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP2D6 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP2D6. This interaction has not been reported in humans.

Here's the wrinkle with this combo product: the hydrocodone part is actually a prodrug. Your liver uses an enzyme called CYP2D6 to turn some of it into hydromorphone, which is a stronger pain reliever. Lab studies (in test tubes, not people) suggest concentrated hibiscus extract might slow that same enzyme down. In theory, that could mean a little less of the strong active form, so you might notice slightly weaker pain relief from the hydrocodone side. The ibuprofen portion isn't really affected this way.

Honestly, this is a theoretical concern only, and it has never been reported in actual patients. An occasional cup of tart hibiscus tea is very unlikely to matter. If you're using a concentrated hibiscus supplement daily, just mention it to your pharmacist or doctor.

This combo pill pairs ibuprofen (an anti-inflammatory) with hydrocodone (an opioid painkiller). Hydrocodone is partly handled by a liver enzyme called CYP3A4. In a test-tube study, hibiscus extract slowed that enzyme down a bit. In theory, that could let hydrocodone linger a little longer, which might mean stronger or longer effects like extra drowsiness, dizziness, constipation, or slowed breathing.

Here's the reassuring part: this has never actually been seen in people, and it's only a minor, theoretical concern. A cup of tart hibiscus tea is very unlikely to cause trouble. Concentrated hibiscus supplements are the bigger question mark. If you take this medicine regularly and want to use hibiscus, just run it by your pharmacist or doctor first.
