Here's the gist for your Isturisa (osilodrostat). Your body partly clears this medicine using a liver enzyme called CYP3A4. In lab dish studies, hibiscus extract appears to slow that same enzyme down. In theory, if osilodrostat is cleared more slowly, a bit more could linger in your system, which might nudge up its effects and side effects, things like low cortisol symptoms (fatigue, nausea, dizziness, low blood pressure), headache, or swelling.

I want to be honest about how weak this is. It's only a test-tube finding, has never been seen in real people, and it's rated minor. An occasional cup of tart hibiscus tea is unlikely to matter. Still, before using concentrated hibiscus supplements, run it past your doctor or pharmacist, since hibiscus can also lower blood pressure on its own.

Here's the honest picture with hibiscus and Isturisa (osilodrostat). In a test tube, hibiscus extract has been shown to slow down an enzyme called CYP2B6, which is one of the pathways your body uses to clear osilodrostat. If that slowing happened in people, the drug could build up a bit more than expected, which in theory might nudge up its usual effects, like lower cortisol, fatigue, nausea, swelling, low potassium, or dizziness.

The important part: this is only a lab-based idea and has never actually been reported in humans. It's rated minor. A cup of tart hibiscus tea is very unlikely to matter, but if you're using concentrated supplements, just give your pharmacist or doctor a quick heads-up first.
