Siltuximab falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hibiscus Sabdariffa interacts with.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP3A4 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP3A4. This interaction has not been reported in humans.

Here's the good news on this pairing. Siltuximab (Sylvant) is a monoclonal antibody, which is a large protein medicine given by infusion. Antibodies like this are broken down into amino acids by your body, not processed through the liver's CYP3A4 enzyme the way many pills are. So even though lab studies suggest hibiscus might slow CYP3A4, that pathway isn't really how siltuximab is cleared.

This interaction is theoretical and comes only from test-tube research, with nothing reported in actual people. In practice, drinking hibiscus tea is very unlikely to change how siltuximab works. Still, since you're being treated for Castleman's disease, it's smart to mention any concentrated hibiscus supplements to your care team before starting them.

Hibiscus sabdariffa is a tart, cranberry-flavored plant most often consumed as a tea, and its best-studied use is for mildly lowering blood pressure. What Is Siltuximab Injection Used For?

Brand name: Sylvant

Sylvant (siltuximab) injection is approved to treat multicentric Castleman's disease (MCD) — a rare disorder involving abnormal lymph node growth driven by an overactive immune signal called IL-6. It is specifically approved for patients who test negative for both HIV and HHV-8 (human herpesvirus-8).

Sylvant has not been studied in patients with MCD who are HIV-positive or HHV-8-positive, because it does not block the form of IL-6 produced by those viruses.
