Thiotepa falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects drugs metabolized by CYP3A4.
In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.

Here's the basics with these two together. Higenamine, in lab dishes, slowed down a liver enzyme called CYP3A4 by about 21%. That same enzyme helps process thiotepa, a chemotherapy drug you'd receive for ovarian, breast, or bladder cancer. So in theory, if that enzyme is slowed, thiotepa could linger a bit longer and its effects (and side effects like low blood counts, nausea, or mouth sores) could feel stronger.

I want to be honest, though: this concern comes only from test-tube research, not from real people, so we don't actually know if it matters in the body. Even so, thiotepa is serious chemo, and higenamine itself can stress the heart. I'd skip the supplement and check with your oncology team first.

Higenamine is a plant-based stimulant compound added to many weight-loss and pre-workout supplements, but there is very little human evidence that it works and real concerns about heart and blood pressure effects. About Thiotepa Injection

What Is Thiotepa Injection Used For?

Brand names: Thioplex, Tepadina

Tepadina (thiotepa) injection is used for several purposes. It treats adenocarcinoma (a type of cancer) of the breast or ovary. It is also used to control malignant effusions — abnormal fluid buildup in body cavities caused by cancer spreading to those areas. For bladder cancer, Tepadina treats superficial papillary carcinoma of the urinary bladder.

Tepadina is also used in pediatric patients with class 3 beta-thalassemia (a severe inherited blood disorder). In those patients, it is given as part of a preparative regimen — a conditioning treatment to reduce the risk of graft rejection before an allogeneic stem cell transplant (a transplant using donor stem cells), alongside the chemotherapy drugs busulfan and cyclophosphamide.
