Thiotepa falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the short version: the worry on paper is that red clover's isoflavones might block an enzyme called CYP3A4, and Thiotepa is partly handled by that enzyme. If clearance slowed, more active drug could build up. With a chemotherapy agent like Thiotepa, that theoretically could mean stronger effects, including more bone marrow suppression (low blood counts), nausea, and infection risk.

But I want to keep this in perspective. This is mostly a test-tube concern. When researchers actually gave people standardized red clover for two weeks, it did not change levels of a CYP3A4 test drug. So a real-world problem is considered unlikely. Still, anyone on cancer treatment should clear every supplement with their oncologist or pharmacist first.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. About Thiotepa Injection

What Is Thiotepa Injection Used For?

Brand names: Thioplex, Tepadina

Tepadina (thiotepa) injection is used for several purposes. It treats adenocarcinoma (a type of cancer) of the breast or ovary. It is also used to control malignant effusions — abnormal fluid buildup in body cavities caused by cancer spreading to those areas. For bladder cancer, Tepadina treats superficial papillary carcinoma of the urinary bladder.

Tepadina is also used in pediatric patients with class 3 beta-thalassemia (a severe inherited blood disorder). In those patients, it is given as part of a preparative regimen — a conditioning treatment to reduce the risk of graft rejection before an allogeneic stem cell transplant (a transplant using donor stem cells), alongside the chemotherapy drugs busulfan and cyclophosphamide.
