Red Clover and Lansoprazole

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Red Clover interacts with whole categories of medication. Lansoprazole falls into 2 of those categories, so it can interact with Red Clover in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

1 of 2

Why this applies to Lansoprazole

Lansoprazole falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase the levels and clinical effects of drugs metabolized by CYP2C19.
In vitro evidence suggests that red clover weakly inhibits CYP2C19. This interaction has not been reported in humans.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Here's the gist with lansoprazole (Prevacid) and red clover. Your liver uses an enzyme called CYP2C19 to break lansoprazole down. In a test tube, red clover's plant compounds appear to weakly slow that same enzyme. If that happened in your body, a bit more lansoprazole could linger, which in theory might mean slightly stronger or longer effects, or minor side effects like headache, nausea, or diarrhea.

The honest part: this is only lab-dish evidence, it has never actually been reported in people, and it's rated minor and unlikely. So I wouldn't lose sleep over it. Still, if you'd like to add red clover, it's worth a quick check with your pharmacist or doctor, especially given red clover's hormone-like activity.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Lansoprazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.
Severity Minor Likelihood Unlikely Evidence B

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: this combination is very low on my worry list. The idea on paper is that red clover's isoflavones might slow down a liver enzyme called CYP3A4, which helps clear lansoprazole. If that happened, you could in theory end up with a bit more lansoprazole in your system. But lansoprazole is an active drug (not a prodrug), and it mostly leans on a different enzyme, CYP2C19, so CYP3A4 isn't the main player here.

More importantly, a real clinical study using standardized red clover found no meaningful effect on a CYP3A4 test drug. So any extra effect, like mild headache or stomach upset, is unlikely. Still, it's always smart to run your supplement list past your pharmacist or doctor, especially given red clover's hormone-like activity.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.

About Red Clover

What is Red Clover?

Red Clover
  • Menopause symptoms (hot flashes)
  • High cholesterol
  • Bone health/osteoporosis
  • Skin and hair concerns
  • Women's health

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. Because of its hormone-like effects, it's not for everyone—talk with your pharmacist or doctor before trying it, especially if you have a hormone-sensitive condition.

About Lansoprazole

What Is Lansoprazole Used For?

Brand name: Prevacid

Lansoprazole is prescribed for a range of conditions where excess stomach acid is the problem. It treats active stomach and duodenal ulcers (sores in the stomach or upper small intestine), helps heal ulcers caused by NSAID pain relievers, and can reduce the risk of those ulcers coming back.

It's also used for gastroesophageal reflux disease (GERD), erosive esophagitis (acid damage to the esophagus), and rare conditions where the stomach makes far too much acid — like Zollinger-Ellison syndrome. When combined with antibiotics, it helps wipe out H. pylori bacteria that cause recurring ulcers.
