Why this applies to Imatinib

Imatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the short version: your body clears imatinib (Gleevec) using a liver enzyme called CYP3A4. In a test tube, red clover looked like it might slow that enzyme down a bit. If that actually happened in people, imatinib could build up slightly and you might notice more of its usual side effects, things like fluid retention and puffiness around the eyes, nausea, muscle cramps, or fatigue.

The reassuring part is that the real-world evidence doesn't back this up. When researchers gave people red clover isoflavones and then a CYP3A4 'test' drug, blood levels didn't change. So this is largely theoretical. Still, imatinib is a serious cancer medicine, so loop in your oncology pharmacist or doctor before adding red clover.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. What Is Imatinib Used For?

Brand name: Gleevec

Imatinib treats a range of blood cancers and solid tumors. It is used for Philadelphia chromosome positive chronic myeloid leukemia (Ph+ CML) in adults and children — whether newly diagnosed or in more advanced phases — and for Philadelphia chromosome positive acute lymphoblastic leukemia (Ph+ ALL) in both adults and pediatric patients.

It is also used for Kit-positive gastrointestinal stromal tumors (GIST), including after surgical removal, and for several rarer conditions: myelodysplastic/myeloproliferative diseases (MDS/MPD) linked to specific gene changes, aggressive systemic mastocytosis (ASM), hypereosinophilic syndrome (HES) and/or chronic eosinophilic leukemia (CEL), and unresectable, recurrent, or metastatic dermatofibrosarcoma protuberans (DFSP).
