Red Clover and Lenacapavir

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Lenacapavir

Lenacapavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Lenacapavir (Sunlenca) is a long-acting HIV medicine that your liver partly handles through an enzyme called CYP3A4. The thinking here is that red clover's plant compounds (isoflavones) might slow that enzyme a little, which in theory could let lenacapavir build up slightly and raise the chance of side effects. But this is mostly lab-dish data, and it's worth keeping in perspective.

Here's the reassuring part: in an actual human study, red clover did not change the levels of a sensitive CYP3A4 test drug at all. So a real-world effect on lenacapavir is considered unlikely and minor. Lenacapavir is too important to your HIV treatment to leave anything to chance, though, so let your pharmacist or doctor know before adding red clover.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Red Clover

What is Red Clover?

Red Clover
  • Menopause symptoms (hot flashes)
  • High cholesterol
  • Bone health/osteoporosis
  • Skin and hair concerns
  • Women's health

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. Because of its hormone-like effects, it's not for everyone—talk with your pharmacist or doctor before trying it, especially if you have a hormone-sensitive condition.

About Lenacapavir Oral

What Is Lenacapavir Oral Used For?

Brand name: Sunlenca

Sunlenca tablets are used in two situations: to treat HIV-1 infection in adults whose virus has become resistant to multiple antiretroviral drugs and whose current regimen is failing — always in combination with other antiretrovirals — and as a short oral loading dose to start the BIXLENVO complete HIV-1 treatment regimen in adults who are already virologically suppressed (virus undetectable) and switching from a stable antiretroviral regimen.

Yeztugo tablets are used as part of HIV-1 pre-exposure prophylaxis (PrEP) — helping HIV-negative people reduce their risk of getting HIV-1. The oral tablet form is used during initiation of Yeztugo, and also as a temporary oral bridge if a scheduled injection must be delayed more than two weeks.
