Bupivacaine, Meloxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ipriflavone interacts with.

Ipriflavone is thought to competitively inhibit cytochrome P450 2C9 (CYP2C9). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP2C9 include amitriptyline (Elavil), diazepam (Valium), estradiol (Estrace), tacrine (Cognex), verapamil (Calan), warfarin (Coumadin), zileuton (Zyflo), and others.

The Zynrelef kit combines bupivacaine (a numbing medicine placed in the surgical wound) with meloxicam, an anti-inflammatory pain reliever. The meloxicam part is cleared by a liver enzyme called CYP2C9. Ipriflavone is thought to slow that enzyme down, so in theory meloxicam could hang around longer and build up to higher levels.

If that happened, you might see more of meloxicam's usual side effects, like stomach upset, heartburn, a higher risk of stomach bleeding, fluid retention, or kidney strain. I want to be honest though: this concern comes only from lab studies, not real people, so it's quite theoretical. Since Zynrelef is a one-time dose given during surgery, the overlap is brief. Still, mention any supplements to your surgical team or pharmacist beforehand.
