Limonene and Esomeprazole

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2C19 (CYP2C19) INHIBITORS

Why this applies to Esomeprazole

Esomeprazole falls under CYTOCHROME P450 2C19 (CYP2C19) INHIBITORS — the group Limonene interacts with.

There's preliminary evidence that limonene might be a substrate for cytochrome P450 2C19 (CYP2C19). So far, this interaction has not been reported in humans. However, watch for an increase in the limonene levels when it is taken with drugs that inhibit CYP2C19. Some drugs that inhibit CYP2C19 include cimetidine (Tagamet), fluvoxamine (Luvox), omeprazole (Prilosec); ticlopidine (Ticlid), topiramate (Topamax), and others.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist of this one: your esomeprazole (Nexium) is one of the medicines that can block an enzyme called CYP2C19. Early lab work suggests that same enzyme may help your body break down limonene. So in theory, esomeprazole could slow the clearance of limonene and let a little more of it build up. Notice the direction here: it's the supplement that may rise, not your Nexium.

The important part is that this has only been seen in test tube and animal studies, never reported in real people. Limonene from a slice of orange or a squeeze of lemon is nothing to worry about. With concentrated limonene capsules it's worth a quick chat with your pharmacist, but this is a low-level, theoretical concern, not an alarm.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

This page covers Limonene only. If you take other supplements — a multivitamin, a sleep aid, something for joints — the Esomeprazole page lists 154 supplements it has a documented interaction with, worst first, each one explained.

By severity: 3 major · 102 moderate · 49 minor.

From our pharmacist’s overview on the Esomeprazole page

“St. John's wort can lower the amount of esomeprazole (Nexium) in your blood by speeding up how the liver clears it, which can leave heartburn or ulcer treatment falling short.”

About Esomeprazole

What Is Esomeprazole Used For?

Brand name: Nexium

Esomeprazole lowers stomach acid. Prescription oral products are used for short-term healing of erosive esophagitis (damage to the esophagus lining from acid), keeping it healed, and treating heartburn and other symptoms of GERD (gastroesophageal reflux disease). They are also used to lower the risk of NSAID-associated stomach ulcers in adults at risk, and to treat pathological hypersecretory conditions such as Zollinger-Ellison syndrome in adults (long-term treatment). With amoxicillin and clarithromycin, they treat H. pylori infection to reduce the risk of duodenal ulcer coming back.

Esomeprazole sodium for injection (intravenous) is used for short-term treatment of GERD with erosive esophagitis when oral treatment is not possible or appropriate. It is also used in adults to reduce the risk of rebleeding of stomach or duodenal ulcers after endoscopic treatment of acute bleeding. Over-the-counter esomeprazole products treat frequent heartburn (2 or more days a week) and are not meant for immediate relief.
