Why this applies to Azilsartan

Azilsartan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Limonene interacts with.

There's preliminary evidence that limonene might be a substrate for cytochrome P450 2C9 (CYP2C9), as well causing its induction. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C9 in patients taking limonene. Some drugs metabolized by CYP2C9 include nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic), and piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarin (Coumadin); glipizide (Glucotrol); losartan (Cozaar); and others.

Here's the gist with azilsartan (Edarbi) and limonene. Your blood pressure pill is partly handled by a liver enzyme called CYP2C9. Early lab work hints that limonene might speed that enzyme up (induce it). If that happened in people, your body could clear azilsartan a little faster, which in theory could make it work less well, so your blood pressure might not be controlled as tightly.

The important part: this is only test-tube and animal data, and it has not been seen in real patients. Splashing lemon or orange in your food is not a worry. Concentrated limonene capsules are the bigger question mark. If you use them regularly, just check your blood pressure at home and mention it to your pharmacist or doctor.

Azilsartan medoxomil tablets — sold as both the generic Azilsartan Medoxomil and the brand Edarbi — are approved to treat high blood pressure (hypertension) in adults. Bringing blood pressure down lowers the risk of serious cardiovascular events, especially stroke and heart attack.

The medication can be used on its own or alongside other blood pressure medicines. It is not a cure for high blood pressure, and managing the condition fully often involves lifestyle steps like limiting sodium, exercising, and quitting smoking.
