Acetaminophen, Butalbital, Caffeine, Codeine falls under METHYLXANTHINES — the group Lithium interacts with.

Theoretically, taking lithium supplements with methylxanthines might decrease lithium levels.
Xanthines such as aminophylline, caffeine, and theophylline (Theo-Dur, Theo-24, others) might increase the clearance of lithium. It is unclear if this interaction would be clinically significant with the smaller doses found in lithium supplements.

Here's the piece that matters for this combo product: it contains caffeine, and caffeine belongs to a group called methylxanthines. On paper, caffeine can nudge your kidneys to clear lithium a little faster, which could lower your lithium level. If lithium drops too low, it may not control your mood as well as it should.

The honest truth is this is a theoretical concern based on how these substances behave, not strong human studies. The small amount of caffeine in one Fioricet-type tablet is unlikely to swing things much, and your overall daily caffeine (coffee, tea, soda) usually matters more. The codeine, acetaminophen, and butalbital pieces don't drive this particular interaction. If you take lithium regularly, just mention it to your pharmacist or doctor so your levels can be watched.
