The piece worth flagging in this combo product is the caffeine. Caffeine is a methylxanthine, and on paper it can nudge your kidneys to clear lithium a little faster. That could mean your lithium level drifts down, which for a mood-stabilizing dose might make it work less well. The other ingredients (acetaminophen, chlorpheniramine, hydrocodone, phenylephrine) are not the players here.

Keep in mind this is a theoretical concern based on how caffeine behaves, not strong human data, and the tiny lithium amounts in supplements may not matter much. Bigger picture: if you take prescription lithium, swings in your level are something to watch closely. Lithium is a naturally occurring metal that, in prescription form (lithium carbonate or citrate), is an FDA-approved, well-proven treatment for bipolar disorder.
