Neem + Cabotegravir, Rilpivirine

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Cabotegravir, Rilpivirine
Cabotegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Cabenuva (the long-acting cabotegravir and rilpivirine injection). The rilpivirine part is partly cleared by a liver enzyme called CYP3A4. In lab dishes, neem leaf extract seems to slow that same enzyme down. In theory, that could let rilpivirine build up a bit, which might mean more side effects like headache, nausea, dizziness, sleep trouble, or mood changes.
I want to be straight with you though: this is only test-tube data and has never actually been reported in a real person, so the real-world risk is likely small. Because Cabenuva keeps your HIV under control, I would not change anything on your own. Just run the neem idea by your HIV pharmacist or doctor first so they can keep an eye on you.

About Neem
What is Neem?
- Skin conditions
- Dental and gum health
- Antimicrobial and antiparasitic
- Digestive upset
- Blood sugar support
- Insect repellent/pesticide
Neem is a tree from India used for centuries in traditional medicine, especially for skin, dental, and antimicrobial purposes. Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. Neem oil and seed extracts can be toxic if swallowed, especially in children, so use with caution and talk to your pharmacist or doctor first.
About Cabotegravir and Rilpivirine Injections
What Is Cabotegravir and Rilpivirine Injections Used For?
Brand name: Cabenuva
CABENUVA (cabotegravir + rilpivirine) is used to treat HIV-1 infection in adults and adolescents 12 years and older who weigh at least 35 kg (about 77 lbs). It is not a starter treatment — it's designed for people who are already virologically suppressed, meaning their HIV is currently undetectable on a stable daily pill regimen and who have no history of treatment failure.
The goal is to keep the virus fully suppressed while freeing the person from taking a pill every day. Because it stays active in the body for weeks after each injection, strict adherence to the injection schedule is essential.
