Why this applies to Dapsone
Dapsone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with neem and your dapsone. Dapsone is partly cleared by a liver enzyme called CYP3A4. In test tube studies, neem leaf extract seems to slow that enzyme down. If that holds true in real life, your body would clear dapsone a little slower, and the drug could build up. That could mean stronger effects but also more side effects, like headache, nausea, or in some folks a drop in red blood cells (anemia) that leaves you tired or short of breath.
I'll be honest though: this is only seen in lab studies, not in actual people, so it's more of a theoretical concern than a proven one. Still, since neem is concentrated in supplement form, I'd run it by your pharmacist or doctor before combining the two.

Dapsone has two main roles, depending on the product.
- Dapsone tablets (by mouth): dermatitis herpetiformis and all forms of leprosy, except cases of proven dapsone resistance.
- Dapsone gel (on the skin): acne vulgaris. Aczone Gel 7.5% and Dapsone gel 7.5% are for patients 9 years and older.
