Here's the simple version: noni juice may have a mild blood-pressure-lowering effect of its own. Your combo pill, amiloride with hydrochlorothiazide, already works to bring blood pressure down by helping your kidneys flush out extra salt and water. Stack the two together and the numbers could drift lower than you want.

If that happens, you might feel dizzy or lightheaded, especially when standing up quickly, along with possible tiredness or feeling faint. I'll be honest with you though, this is mostly theoretical and based on weak, anecdotal evidence, so it's not something to panic over. Just keep an eye on how you feel, and chat with your pharmacist or doctor before adding noni so they can help you monitor things.

Theoretically, combing noni and a potassium-sparing diuretic might increase the risk of hyperkalemia.
Noni juice contains significant amounts of potassium, about 6 mEq/100 mL juice. This may increase the risk for hyperkalemia when used in conjunction with potassium-sparing diuretics, which can also increase potassium levels.

Here's the thing to keep in mind with your water pill. The amiloride part of Moduretic is potassium-sparing, meaning it tells your kidneys to hold onto potassium rather than flush it out. Noni juice happens to be fairly rich in potassium itself (roughly 6 mEq per 100 mL). So when you pair the two, you could nudge your potassium a little too high, a condition we call hyperkalemia.

If potassium climbs too far, you might feel muscle weakness, tingling, or an irregular heartbeat, though that's uncommon. I'll be honest, this concern is mostly theoretical and based on anecdotal reports, so I don't want to alarm you. Still, it's worth a quick chat with your pharmacist or doctor, especially if you have any kidney trouble.

What Is Amiloride and Hydrochlorothiazide Used For?

Brand names: Moduretic, Amilzide, Moduret 25, Amil-Co

Amiloride hydrochloride and hydrochlorothiazide tablets are used for patients with high blood pressure (hypertension) or congestive heart failure who tend to develop low potassium (hypokalemia) when taking a thiazide or other diuretic alone — or for whom keeping potassium levels in a normal range is especially important, such as people taking digoxin or those with significant heart rhythm problems.

The tablets may be used on their own or alongside other blood pressure medicines, such as methyldopa or beta blockers. This combination product is not intended as the first-choice treatment for swelling (edema) or high blood pressure unless low potassium is a specific concern.
