These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Noni interacts with whole categories of medication. Amlodipine Besylate, Benazepril falls into 2 of those categories, so it can interact with Noni in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
ANTIHYPERTENSIVE DRUGS
1 of 2Why this applies to Amlodipine Besylate, Benazepril
Amlodipine Besylate, Benazepril falls under ANTIHYPERTENSIVE DRUGS — the group Noni interacts with.
Preliminary clinical research suggests that drinking noni juice can reduce blood pressure in individuals with hypertension.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Lotrel pairs two blood pressure medicines: amlodipine (a calcium channel blocker) and benazepril (an ACE inhibitor). Both work to lower your blood pressure. Here's the thing about noni: some early research suggests drinking noni juice may nudge blood pressure down a bit on its own. So when you stack noni on top of Lotrel, the two could add up and push your pressure a little too low.
What might that feel like? Dizziness, lightheadedness, feeling faint when you stand up, or being unusually tired. Honestly, this is based on weak, anecdotal evidence, so it's far from guaranteed. Still, if you'd like to try noni, check in with your pharmacist or doctor first, and don't change your Lotrel dose on your own.

ACE INHIBITORS (ACEIs)
2 of 2Amlodipine Besylate, Benazepril also falls under ACE INHIBITORS (ACEIs).
Noni juice contains significant amounts of potassium, about 6 mEq/100 mL juice. This may increase the risk for hyperkalemia when used in conjunction with ACE inhibitors, which can also increase potassium levels.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know about Lotrel: it's a combo pill, and the benazepril half is an ACE inhibitor. One quirk of ACE inhibitors is that they tend to nudge your potassium level up a bit. Noni juice happens to be fairly rich in potassium, so drinking a lot of it on top of your benazepril could stack the two together and push potassium higher than you'd want.
When potassium climbs too high (a state called hyperkalemia), it can cause symptoms like muscle weakness, tingling, or an irregular heartbeat. I'll be honest though, this is mostly a theoretical concern based on anecdotal reports, not strong studies. An occasional glass is unlikely to be a problem for most folks. Just give your pharmacist or doctor a heads-up before using noni regularly, especially if you have kidney issues.

From our pharmacist’s overview on the Amlodipine Besylate, Benazepril page
“Grapefruit sits high on the worry list for anyone taking Lotrel (amlodipine besylate and benazepril), a two-in-one blood pressure pill. It blocks CYP3A4, the enzyme that clears amlodipine, so drug levels can climb, and it is one of nine major-rated interactions among the 257 supplement and herbal ingredients on file.”

Noni is a tropical fruit used widely in folk medicine and sold mostly as a juice or supplement, but solid human evidence for its many claimed benefits is limited.
