Here's the simple version: your Monopril HCT combines two blood pressure medicines, an ACE inhibitor (fosinopril) and a water pill (hydrochlorothiazide). Some early research suggests noni juice itself can gently lower blood pressure. So when you add noni on top of a medicine that's already doing that job, the two may stack and push your numbers a little too low.

If that happens, you might feel dizzy, lightheaded, tired, or woozy when standing up. Keep in mind this caution rests on weak, anecdotal evidence, so it's a possible concern, not a sure thing. An occasional glass is unlikely to cause trouble for most people, but please check with your pharmacist or doctor before using noni regularly, and don't change your medication on your own.

Theoretically, combining noni and ACE inhibitors might increase the risk of hyperkalemia.
Noni juice contains significant amounts of potassium, about 6 mEq/100 mL juice. This may increase the risk for hyperkalemia when used in conjunction with ACE inhibitors, which can also increase potassium levels.

Here's the thing to keep in mind with your Monopril HCT. The fosinopril part is an ACE inhibitor, and one of its known effects is nudging your potassium level upward. Noni juice happens to be fairly rich in potassium (roughly 6 mEq per 100 mL), so drinking a lot of it on top of fosinopril could push potassium higher than you want. The hydrochlorothiazide piece actually tends to lower potassium, which softens the worry a bit, but it doesn't cancel it out.

Now, this is really a theoretical concern based on anecdotal reports, not strong studies, so try not to panic. Very high potassium can cause muscle weakness, tingling, or an irregular heartbeat. If you'd like to enjoy noni regularly, check with your pharmacist or doctor first, especially if you have any kidney trouble.
