Here's the thing to keep an eye on with your Zestoretic (lisinopril plus hydrochlorothiazide): both of these medicines already work to lower your blood pressure. Some early research suggests noni juice may nudge blood pressure down a bit on its own. So when you stack the two together, the worry is that your numbers could dip a little too low.

If that happens, you might feel dizzy or lightheaded, especially when standing up, or notice tiredness, headache, or feeling faint. I want to be honest though, this is based on anecdotal reports and small studies, not strong proof, so it's more of a 'watch and be aware' situation than a hard 'never combine.' If you'd like to try noni, please run it by your doctor or pharmacist first so they can keep tabs on your readings.

Theoretically, combining noni and ACE inhibitors might increase the risk of hyperkalemia.
Noni juice contains significant amounts of potassium, about 6 mEq/100 mL juice. This may increase the risk for hyperkalemia when used in conjunction with ACE inhibitors, which can also increase potassium levels.

Here's the thing about your blood pressure combo: the lisinopril part already nudges your potassium upward, which is just how ACE inhibitors work. Noni juice happens to be fairly rich in potassium (roughly 6 mEq per 100 mL), so drinking a lot of it on top of lisinopril could theoretically push your potassium higher than you want. Too much potassium (hyperkalemia) can cause muscle weakness, tingling, or in serious cases an irregular heartbeat.

The good news is that the hydrochlorothiazide actually tends to lower potassium, so it offsets things a bit. This concern is based on weak, anecdotal evidence, and an occasional small glass of noni is unlikely to be a problem. Still, if you regularly take potassium supplements or have kidney issues, check with your pharmacist or doctor first.

What Is Lisinopril and Hydrochlorothiazide Used For?

Brand names: Zestoretic, Prinzide

Lisinopril and hydrochlorothiazide tablets — sold as the brand Zestoretic and as the generic combination — are used to treat high blood pressure (hypertension). Lowering blood pressure reduces the risk of serious cardiovascular events such as stroke and heart attack.

This is a fixed-dose combination product and is not intended as a first-line (initial) treatment for high blood pressure. It is typically prescribed for patients whose blood pressure is not adequately controlled on a single medication alone.
