Abiraterone falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the connection to keep in mind with abiraterone (Zytiga): this medicine is already known to stress the liver, and your care team checks your liver enzymes regularly while you take it. Padang cassia is a cassia-type cinnamon that naturally contains coumarin, a compound that has caused liver trouble in animal studies and, at very high doses, in some people too.

So the worry isn't a chemical tug-of-war over how abiraterone is broken down. It's simply piling one possible liver stressor on top of another. The sprinkle of cinnamon in your oatmeal won't matter, but concentrated cinnamon capsules could. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please check with your pharmacist or oncologist before using any cinnamon supplement.

Abiraterone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for anyone on abiraterone (Zytiga) who's curious about cinnamon supplements. Your liver uses an enzyme called CYP3A4 to help clear abiraterone from your body. In lab tests, a concentrated extract of Padang cassia slowed that same enzyme down. If that happened in a real person, abiraterone could build up a little more than expected, which in theory could mean more of its usual side effects like high blood pressure, swelling in the legs, low potassium, or fatigue.

The important thing: this is only a test-tube finding, not something proven in people, so I wouldn't panic. Sprinkling cinnamon on your oatmeal is no concern. Concentrated cinnamon capsules are a different story, so check with your oncologist or pharmacist before adding them.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

About Abiraterone

What Is Abiraterone Used For?

Abiraterone acetate tablets are used to treat two types of advanced prostate cancer: metastatic castration-resistant prostate cancer (CRPC — cancer that has spread and keeps growing despite low testosterone levels) and metastatic high-risk castration-sensitive prostate cancer (CSPC — cancer that has spread but still responds to hormone-lowering treatment). It is always taken together with a corticosteroid — most products pair with prednisone, while Yonsa is specifically indicated with methylprednisolone.

All products — including Abiraterone, Abiraterone Acetate, Abirtega, Yonsa, and Zytiga — are taken by mouth as a tablet. Yonsa is approved only for metastatic CRPC, while the other products (Abiraterone, Abiraterone Acetate, Abirtega, and Zytiga) are approved for both metastatic CRPC and metastatic high-risk CSPC.
